Kid Fun Yoga takes all reasonable care to ensure that its classes are fun and safe; however, I acknowledge and fully understand that my child will be engaging in physical activities that may involve some risk of injury, which may occur as a result from participating in the class. I assume these risks and hold harmless Kid Fun Yoga owners and teachers from any claim, cause of action or liability for damage arising from any personal injury, illness, or other heath issue caused to my child by my child’s participation in any Kid Fun Yoga programs or classes. I understand that it is my responsibility to consult a physician prior to and regarding my child’s participation in yoga classes. I will inform my child’s yoga teacher should there be any change in my child’s medical condition. I understand that it may be necessary for the yoga teacher to appropriately reposition my child, to safeguard him/her from injury, and I consent to this.

Photo Release Consent

Dear Families, We are excited to share that we will be taking class pictures during the semester of Kid Fun Yoga After School Enrichment Program. These photos may be emailed to the email address on your registration form and will continue to be updated through the semester for you to enjoy. We seek your consent allowing Kid Fun Yoga to take class pictures of your student during kids yoga classes.
